This Critical incident reporting guideline and form should be used by the following programs: Children and Parenting Support services, Family and Relationship Services (excluding the Specialised Family Violence Services component), Reconnect, and Communities for Children Facilitating Partner (CfC FP) services.

Family Mental Health Support Services (FMHSS) should use the Incident Report Form at Appendix H of their Operational Guidelines (effective 1 July 2021).
