Aim of publication

This good practice guide promotes sharing of some initiatives that support young people transitioning from out-of-home care to independence across Australia in 2011-12.  Improving the experience of young people transitioning to independence is one of the specific actions under the National Framework for Protecting Australia’s Children 2009–2020 (the National Framework).

Who is it for?

This resource is for policy makers, non-government organisations and service providers supporting young people in, and leaving, out-of-home care.  It contains information that may also be of interest to young people, carers and families.

What does it cover?

The good practice guide is a point in time resource that promotes information sharing and further innovation in supporting young people leaving out-of-home care across State and Territory child protection agencies and non-government organisations.

The guide includes examples of practice relevant to each of the three different phases:

  • preparation
  • transition
  • after care independence.

It also includes examples that traverse the continuum of transitioning to independence, detailing support available throughout the process.
